The Applicant Shall enter his/ her Bank Challan on Online Application Form.
- Application fee for BPS 11 RS 500-/, BPS RS 1000.
- Submit their Application Form along with attested copies of CNIC, Domicile, Educational Documents & Passport Size Photograph at the University of Health Science, Khayaban – e – Jamia, Punjab, Lahore.
- Selected candidates shall have to Provide NOC from the head of their respective Parents’ Institutions.
- No TA/DA shall be admissible for interview.
